All Twelve Data paid plans include access to real-time US stock market data suitable for both personal and commercial use. No additional exchange fees are required to access the default feed. However, some conditions apply depending on your intended use:

External distribution to users requires a US Equities Add-On (read more below).

OTC securities
OTC (over-the-counter) securities are not included by default in any Twelve Data plan. These instruments trade outside of traditional exchanges and require separate licensing.
